Event To Explain How To Take Control Of Your Health And Social Care

AN EVENT aimed at showing how to plan the future of your health and social care will take place in Wigtownshire later this month.

 

Planning Life’s Journey is being organised as part of Transforming Wigtownshire, and will set out the role which can be played by systems such as the Anticipatory Care Plan (ACP) and Power of Attorney (POA).

 

Transforming Wigtownshire Programme Manager Karen Harper said: “This is going to be a fun, relaxed event offering people a chance to see how they can take greater control of their own health and social care.
“We had a fantastic event in Stranraer in the summer called ‘Navigating Life’s Journey’, which saw staff and volunteers who are involved in delivering a range of services take the opportunity to highlight them and explain them to the many people who attended.
“Our event taking place at the Millennium Centre on January 31 will go into more detail on the tools and systems which can help people take more control of their health and social care.  We have engaging presentations from experts who will talk about their experiences, explain what Anticipatory Care Plans and Power of Attorney are and answer your questions.
“We’re going to have key figures involved in delivering the Anticipatory Care Plan coming along to explain how this living document allows someone to set out what they expect when it comes to their treatment and care, and which accompanies them on that journey.
“Meanwhile, our presentation on Power of Attorney will set out this document allows family and loved ones take on a role in helping to guide their loved ones’ health and social care journey when they are perhaps no longer in a position to control this themselves.”

 

Planning Life’s Journey takes place at the Millennium Centre in Stranraer on Friday  January 31 from 10 am to 12.30 pm.

 

This is a free event open for everyone to attend – both the public as well as those people involved in delivering health and social care services.
